Configure EEO questionnaire (Optional)
Configure EEO questionnaires in Workday for Indeed integration.
Who: Workday administrator
Time: ~30 minutes (one-time setup)
Skip this section if your company does not gather EEO data from applicants.
Configure EEO questionnaires in Workday to collect demographic information from job seekers. Indeed supports primary and secondary questionnaire types. Questionnaires attached to a job requisition are displayed automatically to applicants. For Question Body fields, you can include rich text formatting such as bold, lists, and hyperlinks.
Step 1: Create EEO questionnaire

Search for
Create Questionnaireand select the Task. -
In the Name field, enter
Indeed EEO US English. The name must match exactly. Do not attach the questionnaire to any jobs. Indeed displays EEO questions for US jobs only. -
Select OK.
Step 2: Create EEO questions
Indeed supports these Workday EEO question types:
Data collection questions:
- Gender
- Ethnicity (single or multiple select)
- Veteran Status
- Hispanic/Latino
- Terms and Conditions (answer not delivered)
- Disability Status Last Update Date
- Disability Status
Informational questions:
- Personal Data Statement
- Disability Instructions
- Disability Public Burden
- Page Breaks
All question names and possible answers must match the exact text specified in this guide.
To begin creating questions:
After creating Gender, Ethnicity, and Hispanic/Latino questions, you must update localization settings to enable these fields for US candidates. Instructions are included in each question section below.

Select Maintain Questions.

From the Question dropdown, select Create Question.

Configure each question type you need using the instructions below. Questions are listed in the recommended presentation order.
Personal Data Statement

In the Question Name field, enter
Indeed EEO US Personal Data Statement. -
In the Question Body field, enter the Personal Data Statement from your External Career Site settings.

For Question Type, select Text.

Select OK.

In the Question Name field, enter
Indeed EEO US Gender. -
Enter your question body text.

For Question Type, select Multiple Choice - Single Select.

Get the possible answer choices:
- Open a new browser tab and search for
View Reference IDs. - Select the Report.
- In the Business Object field, enter
Gender. - Select OK.

- In the Area field, enter

- Under Localized Field, find the row for Gender (Candidate).
- In the Allow for Countries or Regions column, ensure
United States of Americais in the list. - Ensure the Active checkbox is selected.
- Select OK.
- Open a new browser tab and search for
Ethnicity

In the Question Name field, enter
Indeed EEO US Ethnicity. -
Enter your question body text.

For Question Type, select one of the following:
- Multiple Choice - Single Select if job seekers should select only one choice.
- Multiple Choice - Multiple Select if job seekers can select one or more choices.

If you selected Multiple Choice - Multiple Select, configure your Workday tenant:
- Open a new browser tab and search for
Edit Tenant Setup - Global. - Select the Task.
- Under Personal Data for Workers, find Allow Multiple Ethnicities.
- Ensure
United States of Americais in the Allow in these Countries list. - Select OK.
- Open a new browser tab and search for

Terms and Conditions

Indeed EEO US Terms. -
In the Question Body field, enter the Terms and Conditions from your External Career Site settings.

For Question Type, select Multiple Choice - Single Select.

Add these possible answers:
- The Candidate Consent from your External Career Site settings
- Unused (this can be any value. Indeed does not use it, but Workday requires at least two possible answers)
Page Break
Use page breaks to create different sections of questions.

In the Question Name field, enter
Indeed EEO Page Break. Do not include "US" in the question name for page breaks. -
In the Question Body field, enter
Unused.This can be any value. Indeed does not use it, but Workday requires a question body.

For Question Type, select Text.
Disability Status Last Update Date

In the Question Name field, enter
Indeed EEO US Disability Status Last Update Date. -
Enter your question body text.

Disability Instructions

Indeed EEO US Disability Instructions. -
In the Question Body field, enter the instructions displayed on the Voluntary Self-Identification of Disability page when job seekers apply on your Career Site.

In the Question Name field, enter
Indeed EEO US Disability Status. -
Enter your question body text.

For Question Type, select Multiple Choice - Single Select.

Get the possible answer choices:
- Search for
Integration IDsand select the Report. - In the Business Object field, enter
Self-Identification of Disability Status. - Select OK.
- From the results, select options from the Instance column where the ID ends in
_2026to use as Possible Answer choices.

In the Question Body field, enter the PUBLIC BURDEN STATEMENT displayed on the Voluntary Self-Identification of Disability page when job seekers apply on your Career Site.

For Question Type, select Text.
Finalize questionnaire
After creating all the questions you need:

To make a question required, select the checkbox in the Required column for that question.

To add another question, select the plus icon and repeat the configuration process for that question type.

Select OK to save the questionnaire.
